New Employees

Welcome to FIT! In order to expedite the new hire process, please complete the following forms that make up the New Hire Checklist and bring them with you on the day of your appointment.

All new hires are required bring original documents as we cannot accept photocopies. Additionally, you will need to show us your social security card as proof of your identity.
