The College offers new full-time employees the option to enroll in the health insurance plan described below, or to waive coverage and receive waiver payments. Proof of other health insurance coverage is required for waiver of coverage.
COVERAGE EFFECTIVE DATE: 30 days following hire date
COST: There is no premium cost to employees for either plan described below; the College covers the full premium cost for Individual or Family coverage.
Enrollment Details: Employee portion subject to change. If you are enrolling with family coverage, you may enroll your spouse or domestic partner and children up to the end of the month in which they turn 26. Domestic partner eligibility guidelines and income tax consequences apply; details are available upon request.
AETNA CHOICE POINT-OF-SERVICE II: This plan is a medical and hospital plan which offers you the option of using either in-network or out-of-network providers/hospitals each time you access care. You do not need to elect a primary care physician (PCP) and no referrals are needed for care by specialists. For in-network providers, most eligible services are covered for a co-pay of $25 for a PCP and co-pay of $50 for specialists. When you use out-of-network providers, eligible claims are paid at 80% of reasonable and customary charges after an annual deductible of $500* per individual (maximum of $1,500 for family) is met. WAIVER OF COVERAGE: If you are covered by a medical and hospital health insurance plan from another source, you may elect to waive coverage in FIT's plan and receive a $50 per pay period waiver payment from the College (a total of $1,200 per year). Proof of other health insurance coverage is required.
IMPORTANT NOTICE: If you do not enroll in the health insurance plan within the initial 30-day period following your hire date, you will not have insurance coverage until the next annual open enrollment period which is held in November of each year for a January 1st effective date.
Current employees who wish to enroll in the health plan who have chosen to waive the Aetna benefit and/or employees who have had a change in family status must notify us of that event within the initial 30 day of that event change. Should prompt notification not take place, employee must wait until the open enrollment period in order to make changes which will take effect January 1st of the following year.
