COBRA Rates

Enrollment in FITís health insurance plans is optional. You may enroll your spouse or domestic partner, and your dependent children up to age 26.  For information on Domestic Partner documentation and the possible tax consequences of enrolling a Domestic Partner on the medical plan, please contact the Human Resources Benefits area at extension 73670.  When enrolling in the Aetna-HMO you will be placed in the plan for the state you reside in. 

Please note the enrollment periods for each plan. You will be billed on a quarterly basis for the health insurance plan you choose (the payment will not be deducted from your paycheck).

Part-time Faculty
Following three (3) years of employment, part-time faculty members who teach a minimum of six (6) contact hours per week will be eligible for a partial reimbursement from the College for health insurance premiums:

From Collective Bargaining Agreement, section 35.1.1
There shall be a three (3) year waiting period before reimbursement of health insurance commences and reimbursement will only be given to those part-time classroom faculty who teach a minimum of six (6) hours day, evening and/or weekend, or a combined program.

For those part-time classroom faculty teaching only day hours or a combined program, there shall be a twenty percent (20%) rate per each three (3) hours, prorated. For part-time classroom faculty teaching evening and/or weekend hours only, there shall be a fifteen percent (15%) reimbursement rate per each three (3) hours, prorated, but only when teaching six (6) hours or more.

Additional Reimbursement Information
For enrollees on the HIP HMO or the HIP POS plan, the reimbursement noted above is based on the basic premium rates only (administration fee is not reimbursed).

For enrollees on the Aetna HMO plan, the reimbursement rate noted above will be based on the Aetna rate if the Aetna rate is equal to or less than the HIP POS rate. If at any time the Aetna rate is higher than the HIP POS rate, the reimbursement will be based on the (lower) HIP POS rate.

Staff and Classroom Assistant Employees
You must pay the full premium rates for coverage during the first five (5) years of employment. For years six (6) and seven (7) of employment, part-time staff and classroom assistant employees working eighteen (18) or more hours per week will be eligible for a 35% reimbursement of health insurance premiums from the College, a fifty-percent (50%) reimbursement for years eight (8) through twelve (12), and a sixty-five percent (65%) reimbursement thereafter. The reimbursement is based on the Individual premium rate only (for those enrolled on Family coverage, employee pays the difference between Individual and Family premiums).

HEALTH INSURANCE PLAN OF NEW YORK (HIP)
(The HIP Plans are administered by the UCE of FIT Welfare Fund)
Enrollment Periods: You are eligible to enroll in either the HIP HMO or HIP POS plans described below during your first sixty (60) days of employment or during the regular open enrollment periods which are the month of September and the month of February each year. In addition, a special enrollment period is available to individuals who lose other group health insurance coverage provided application is made within 30 days of the loss of such coverage. For plan information, provider directories, or an enrollment application, please contact the UCE Welfare Fund office at (212)217-3370 or stop by the office which is located on-campus in room B-902.

HIP PRIME HMO (Health Maintenance Organization)
This plan includes an extensive network of health care facilities and physicians in the five boroughs of New York City, plus Nassau, Suffolk, Westchester, Rockland and Orange counties. You must select a HIP participating primary care physician who will coordinate your care and referrals to HIP specialists and hospitals. The plan offers a package of medical, hospital, and prescription benefits. There is no out-of-network coverage with this plan except for emergency care.

HIP POS (Point-of-Service Plan)
Available in the five boroughs of New York City, plus Nassau, Suffolk, Westchester, Rockland and Orange counties. This plan offers a package of medical, hospital, and prescription benefits. The plan allows you to either use out-of-network providers and hospitals or HIP in-network providers and hospitals. However, there is an out-of-pocket annual deductible and co-insurance charges when using out-of-network providers and hospitals.

AETNA HMO Plan (Health Maintenance Organization)
(The Aetna plan is administered by FITís Human Resources Department)
Enrollment Periods: You are eligible to enroll in the Aetna HMO plan described below during your first thirty (30) days of employment or during the annual open enrollment held in the month of November each year for a January 1st effective date. In addition, a special enrollment period is available to individuals who lose other group health insurance coverage provided application is made within 30 days of the loss of such coverage. For plan information or an enrollment application, please contact the Human Resources Benefits area at (212) 217-3670, or stop by the office at 236 W. 27th Street, 11th Floor. This plan includes an extensive network of physicians and hospitals throughout the tri-state area of New York, New Jersey, and Connecticut. You must select an Aetna participating primary care physician who coordinates your care and referrals to specialists and hospitals. The plan offers a package of medical, hospital, and prescription benefits. Under this plan, office visits are covered for a $10 co-pay. There is no out-of-network coverage with this plan except for emergency care.
