Consent Form
Health Information, Concent, and Assumption of Risk Release Form will be emailed to all students after they register and pay for the class for a given semester.
We require parent / guardian to provide personal and health information and consent in order to create a safe and enriching environment for all Precollege Program participants.
Please read the document thoroughly, understanding its contents, before signing and dating in the designated spaces. Your signature will indicate voluntary agreement and understanding of the terms outlined here that will be covered in more detail in the form:
- Health Information we gather essential details about the student's medical history, prescription medications, allergies, and emergency contact information.
- Healthcare Authorization and Release outlines our procedures in the event of illness or injury.
- Assumption of Risk and Release of Liability we acknowledge the inherent safety risks associated with program participation. Your initialing signifies your understanding and acceptance of these risks, releasing FIT from liability while affirming your commitment to assume responsibilities.
- Photography/Video Consent addresses the use of media during the program. Your consent allows FIT to use images and recordings for educational and promotional purposes.
- Behavioral Expectations ensures awareness and agreement with the Precollege Student Conduct Guidelines and Program Protocols. Section F provides information for students with disabilities. If accommodations are needed, please contact FIT's Office of Disability Services (FIT-ABLE).
Only parents / guardians of Precollege students under the age of 18 or Precollege students 18 years of age or older may grant consent for a student's participation in FIT's Precollege Programs.
